Develop a release of patient information compliance checklist (3-4 pages) for hospital staff members to follow. Include an introduction that covers the HIPAA Privacy Rule, proper and improper use of health information, and existing controls to protect patient confidentiality and privacy.Electronic health records and modern communication technologies have made health information increasingly accessible. As a result, maintaining privacy requires the handlers of health information to know and follow release of information (ROI) protocols closely. ROI is also critical to the quality of the continuity of care. ROI also plays a significant role in billing, reporting, and other functions related to patient care processes. ROI procedures must clearly define privacy and confidentiality, often used interchangeably. However, they are distinctly different concepts. Privacy refers to the freedom from intrusion into an individual’s personal information. Confidentiality refers to the sharing of information with unauthorized users. Confidentiality is usually viewed as an ethical responsibility; however, privacy is more of a legal matter. Privacy specialists believe that patient information belongs to patients, and they have a right to protect themselves from the potential impacts of having their information available to providers and other users. However, release of patient information is often needed for providing patient care and serving other purposes. HIPAA establishes the standard to protect patients’ confidential information and specifies the procedures for releasing it.In this assessment you will continue on in your new role as a HIM analyst for Valley City Regional Hospital. You will gather information from the hospital’s risk manager about privacy issues and develop a three- to four-page compliance checklist, including an introduction, for the release of patient information for all hospital staff members to follow. You will want to make sure that your checklist specifically addresses the privacy issues the risk manager shared with you. Demonstration of ProficiencyBy successfully completing this assessment, you will demonstrate your proficiency in the course competencies through the following assessment scoring guide criteria:Competency 1: Describe the purposes and scope of the Health Information Portability and Accountability Act (HIPAA).Explain how the HIPAA Privacy Rule applies to audit violations. Competency 2: Integrate privacy rules and regulations into health information management processes.Outline the release of information process, including procedures, used for highly sensitive data. Distinguish between proper and improper use of health information. Describe the process to de-identify patient information. Describe controls to protect patient confidentiality and privacy.Competency 5: Communicate effectively in a professional and ethical manner.Create a clear, well-organized, professional compliance checklist that is generally free of errors in grammar, punctuation, and spelling.Follow APA style and formatting guidelines for citations and references. PreparationTo successfully complete this second course assessment:View this media piece: Vila Health: Privacy. As you view the media piece, keep in mind privacy requirements and the potential privacy violations presented. Based on the media piece, you will develop a compliance checklist, outlining for staff members the steps they need to take to release patient information. Review the risk report and SWOT analysis you completed in Assessment 1. You may have an opportunity to incorporate elements from some of your previous work into this assessment.InstructionsIn this assessment you will continue on in your new role as a HIM analyst for Valley City Regional Hospital. The director of quality assurance likes the risk report you recently prepared (Assessment 1). Now, he would like you to complete another task. Other leaders have brought to his attention some concerns about whether hospital staff professionals are consistently complying with privacy requirements. He’d like you to gather information from the hospital’s risk manager about this issue. The Vila Health: Privacy media piece will provide information about the risk manager’s insights. Your task will be to develop a three- to four- page compliance checklist, including an introduction, for the release of patient information for all hospital staff members to follow. You will want to make sure that your checklist specifically addresses the privacy issues the risk manager shared with you. Be sure your ROI checklist includes all of the following:What are the major steps hospital staff members need to follow when releasing patient information? How does the HIPAA Privacy Rule apply to each step in the process? What are the proper procedures to follow when de-identifying patient information? Also include an introduction to your checklist. Your introduction needs to address all of the following questions:What is the HIPAA Privacy Rule and how does it apply to the ROI process? What constitutes proper and improper use of health information? What controls does Valley Regional Hospital have in place to protect patient confidentiality and privacy?Note: As a best practice, consider completing the checklist first. As you write the checklist, you may discover information you want to include in your introduction.Additional RequirementsLength: 3- to 4-page double-spaced compliance checklist with an introduction.Format: Times Roman, 12-point type.APA: Follow APA style and formatting guidelines for citations and references. Include a separate works cited page for your references. Writing: Create a clear, well-organized, professional compliance checklist that is generally free of errors in grammar, punctuation, and spelling.

Discussion C DISCUSSION Welcome to the discussion. In this course you will be introduced to a topic in the discussion. The first week of the module you will write about your initial thoughts after reviewing the resources. The initial post should be at least 300 words. During the following week you will reply to at least two of your classmates. The replies should be at least 100 words. See the discussion rubric for more details on grading. Discussions should always be completed prior to “class.” If you are in an online class or an on ground course this is prior to the lesson portion of the week. Are you ready to begin? Read the article in the link below from the Wall Street Journal about the recent contamination of a Florida aquifer by industrial radioactive waste. What are some of the social determinants here that led to this environmental disaster? What effect can such issues have on the health of a community? http://www.wsj.com/articles/florida-residents-sue-mosaic-over-sinkhole-1474644337 TASK 5 SOCIAL AND ENVIRONMENTAL DETERMINANTS OF HEALTH Investigate the CDC site on social and environmental determinants of health Review the CDC Tools for putting SDOH into action https://www.cdc.gov/socialdeterminants/tools/index.htm and https://www.cdc.gov/socialdeterminants/cdcprograms/index.htm Choose several of the examples of programs and tools and review thoroughly to use in the lesson. Determining Community Needs and Services Complete a windshield survey of your neighborhood or a nearby area. Do not go into areas that would be considered unsafe. The intent of the survey is to observe the social and environmental issues in the area as you drive through. You will be amazed at what you can see and the detail that you will observe. If possible, take someone along who can jot down notes. This will take about 30 min to 1 hour to complete sufficiently. When done write up a brief report of your findings with justification of the social and environmental determinants you have observed and how they could affect health either positively or negatively. Windshield survey information Lesson 5 Lesson 5 Activity 1: According to the CDC, health starts “in our homes, schools, workplaces, neighborhoods, and communities. We know that taking care of ourselves by eating well and staying active, not smoking, getting the recommended immunizations and screening tests, and seeing a doctor when we are sick”, (CDC, 2016) In this exercise, you will review the social determinants of health as described by Healthy People 2020, the CDC’s planning framework for health improvement. Look at the various factors described in the graphic below. Think about these from the point of view of where you live; which ones are more or less important. Based on what you found in the windshield survey, discuss with your team each of the points. Activity 2: Using the information discovered/interpreted during the class activity, create an infographic to share with political and community leaders about the more important social determinants of health in your home neighborhood. Activity 3: Determining Needs and Resources Based on the windshield surveys and other information gained begin to determine a plan to assess the needs and resources of the neighbor. If working in a team, choose one neighbor for the concentration of your plan. Choose a neighbor where there is a clear need for public health services. Project Assignment: Begin to develop a plan of action in the event of an outbreak or emergency (natural, bioterrorism, chemicals, radiation, etc) either from within the country or from a global source. What is the immediate response? Who will be involved in the coordination of the response? Who will be the first responders? Are they prepared, trained for different types of events? What will the communication structure be especially in the event that normal communication is affected negatively by the event? https://emergency.cdc.gov/planning/ Submit here

United States Economy: Expansion or Recession? Research Paper
According to the key indicators, such as gross domestic product (GDP), unemployment rate, inflation rate, and interest rates, the US economy may be considered to be healthy. The GDP rate is expected to be between 2 and 3%, which is a normal value. Unemployment level continues to decrease at the natural rate, and there is not much inflation (1,9%). Based on the data presented in the table and graphs, one may conclude that real GDP rises and the unemployment falls, which allows for saying that the US economy is experiencing an expansion. It should also be stated that the economy is above its potential values of GDP ($18,6 trillion compared to $16,7 trillion) and Unemployment (3,7% compared to 4,7%). In September 2018, the interest rate was 2,25% which is expected to increase to 2,5% in December 2018. In the given essay, policy recommendations aimed at maintaining economic growth are discussed. For the current period of expansion to become the longest, the economy will have to continue growing in the next years. However, more economists are concerned with a growing federal debt relative to GDP. On the last fiscal day of 2018, the national debt rose above $21,5 trillion due to increased spending and tax cuts (Richter, 2018). Even though Conservatives in Congress complain that spending mainly remains unchecked which will inevitably lead to an increase in federal debt, Trump and GOP leaders agreed to boost federal spending by $300 billion. Under the current law, the US government may borrow as much as it needs. Starting with March 1, 2019, the debt ceiling will prevent the government from borrowing above the level of debt on that date. The current law on federal spending may not be considered satisfactory. Even if federal debt stops growing, it will displace private capital investment which may lead to negative consequences. Apart from that, if the US economy hits the financial crisis with debt accounting for more than one-third of GDP, economists and politicians will not have many options to choose from. Therefore, fiscal policy should be aimed at decreasing debt-to-GDP ratio. The recommendations presented below discuss what should be done in fiscal policy to decrease the federal debt. The first recommendation is to increase federal investment in education, healthcare, and research and development. This would enable to create a great deal of new jobs (and thus reduce the level of unemployment) and strengthen the American education. Education grants may be given to facilitate teacher preparation and provide financial support to all students to afford higher education. One may note that cutting costs on federal spending is not a forward-looking policy, as it may reduce potential total output and income. Apart from that, it should be highlighted that cutting federal spending in education will reduce the incomes of people who need particular support from the government. Also, taking into account significantly low Treasury interest rates, federal spending should be increased. Get your 100% original paper on any topic done in as little as 3 hours Learn More One may note that it is crucial to increase the return on federal spending as funding may be dissipated. This may be achieved by careful cost-benefit analysis and audit of government subsidies for innovative research projects in order to minimize government pending on unpromising startups. One may suggest that it is important to understand that some categories of federal spending are investments in future income. This is particularly true for benefits for lower-income families, such as healthcare benefits and housing subsidies. These investments should be protected to increase future profits of some young people. This recommendation has been supported by the Center for American Progress (2015) that highlights the importance of investing in infrastructure, innovation, and national service to create jobs. It also states that middle- and low-class families should get affordable higher education and retirement. The second piece of recommendation is that benefits for higher-income retirees should be reduced while benefits for working people and low-income retirees should not. One may foresee that in the next 20 years, spending for Medicare and Social Security will increase in relation to GDP. This may be explained by the gradual aging of the US population, which will lead to a growing number of beneficiaries. The crucial point is that an increase in government spending may be explained not by a general economic enhancing in the federal government but a significant increase in spending for Medicare and Social Security. One may assume that over the last years, people have gained little from the economic growth, in particular, total output. Based on this evidence, the government should aim to raise incomes for people with modest means. Benefits in healthcare programs, such as Medicare and Social Security, should not be cut as this would result in insufficient retirement income for poor people. Therefore, one may state that benefits should be cut for high-income beneficiaries of Social Security and Medicare. One the one hand, this recommendation has certain disadvantages, as transforming Social Security and Medicare would alleviate the connection between one’s taxes and benefits. One may also note that this may make high-income people work and save less. On the other hand, this approach may be of help in restraining federal debt without burdening low- and middle-income Americans. This recommendation has been supported by the American Enterprise Institute (2015) that proposed larger subsidies to be paid to beneficiaries who are in greater financial need or who have higher health risks. Therefore, reducing benefits for high-income retirees may be an important tool in enhancing the US economy. The last recommendation is to raise tax revenue to sustain the US budget in the long term. It is obvious that there is no other possible way to considerably reduce the federal debt than to get more revenue by raising taxes. This recommendation may be supported by the fact that Republicans approved only a few bills on cutting government spending during several years that they controlled the House of Representatives. Therefore, raising tax revenue is still the most efficient way to get additional revenue. There are two ways to start raising tax revenue, one of which is to tax carbon emissions, and another one is to broaden the tax base by reducing deductions and exemptions. Reduction of carbon emissions is approved by both experts on the climate and experts on the economy, though, for different reasons. It was a Republican lawmaker Carlos Curbelo who introduced a bill calling for a carbon tax (Milman, 2018). This would enable to control carbon emissions as well as get additional revenue. Economists state that if tax expenditures that provide financial assistance for certain activities and groups of people are reduced, the US economy will benefit from it. This is because canceling deductions and exemptions will allow for obtaining a considerable federal revenue from the business taxes. Three fiscal policy recommendations aimed at maintaining the US economy and reducing the federal debt have been discussed, the supporting evidence for them has been provided. The recommendations include increasing federal investment, reducing benefits for higher-income retirees, and raising tax revenues. Supporting Materials Table 1. Key Economic Indicators.
