Bow Tie Analysis Example.jpg In my class we examined the Joint Commission’s framework for root cause analysis of sentinel Incidents in health care organizations. For purposes of this project, a critical incident is a key occurrence, but it is not a sentinel event. A bow-tie analysis places the critical incident at the center of the framework and identifies in graphic format the root cause leading up to the incident on the left side of the critical incident. On the right side of the incident, key preventive measures are graphically represented. In one image, you will capture the incident (at the center), the causes for failure in quality, and (on the right) preventive measures a manager can put in place to stop this failure from happening in the future. The final graphic will appear as a bow-tie. One crewmember, named Blue, is found to be one of the killers. Blue’s son, Green, is also one of the crewmembers on the ship. The other crewmembers claim that Green must be the second killer. They make the argument that Blue resorted to killing to improve his survival on the ship (he would have greater access to resources). Since Blue’s behavior lead to increased fitness, his son Green would have inherited this serial killing trait. Would you consider this a Lamarckian or Darwinian view of natural selection? Explain your reasoning. Let’s say that there is a serial killer gene that can be inherited. Several generations of Blue’s offspring inherit the gene. On the planet base, Polus, one of the crewmembers (Orange) develops the serial killing trait due to similar environmental circumstances. Orange is not related to Blue.
