(2017), J.R. has classic symptoms of genital herpes simplex virus infection, or HSV, for which 500,000 new cases occur each year. Most cases of HSV in people remain undiagnosed as disease transmission spreads while a person is infected but asymptomatic.According to Hollier (2018), symptoms of primary infection include genial ulcerations that are painful, hyperesthesia, malaise, myalgia, dysuria, regional adenopathy, and cervical lesions for women. As healthcare providers, it is important also that we rule out differential diagnosis such as syphilis, atypical genital warts, Herpes Zoster, and Bahcet’s disease (Hollier, 2018).Drug therapy for HSV strives to suppress viral symptoms and control outbreaks but cannot eradicate the virus. First-line therapy are systemic agents that are chosen because they inhibit viral DNA replications and include acyclovir (Zovirax), famciclovir (Famvir), and Valacyclovir (Valtrex) (Arcangelo et al., 2017). The specific recommended dosages and length of drug therapy are dependent upon the stage at which a patient is diagnosed. For instance, pharmacotherapy for a patient who presents with their first clinical episode of HSV-1 will be treated with specific doses of first-line therapy for 7 to 10 days, recurrences treated with second-line therapy for 5 days, and an additional third-line dosage therapy for daily suppressive therapy. Parameters for monitoring the success of pharmacological therapy for HSV is the evaluation of symptom relief and resolution of lesions (Arcangelo et al., 2017). Patients should be seen by the healthcare provider within one week to assess for clinical improvement of lesion outbreak. If on a suppressive therapy treatment, patients should be reevaluated in one year. According to Hollier (2018), patients who have recently acquired HSV are at an increased risk for HIV. HIV and additional STD testing should be implemented at first evaluation and a scheduled Pap smear every three years. Patient education for J.R. includes abstinence from sexual activity while genital lesions or prodromal symptoms are present (Arcangelo et al., 2017). Prevention of the spread of this STD while the patient is asymptomatic is very important as is the use of condoms and suppressive therapy. J.R. needs to be aware that recurrent episodes require immediate initiation of drug therapy within one day of prodrome symptoms or lesion outbreak. Additionally, she should be encouraged to inform her partner and be offered emotional support to help the patient cope with this diagnosis. Finally, patients diagnosed with HSV should be informed of potential health complications such as prolonged severe local disease, secondary bacterial infection, increased risk for HIV infection, and increased risk of maternal0fetal transmission during birth (Hollier, 2018).The patient and her partner should both be advised on the importance of abstaining from unprotected sexual intercourse or oral sexual contact during the third trimester, should they become pregnant, in order to decrease the risk of neonatal infection.
