To rebuild mutual respect, we would like to be willing to show respect for each other. A specific way is to minimize sending negative messages (Elder, 2010). In addition to individual time Sammy and her parents can schedule a family time once a or few week, it should be a time of happiness and joyful by throwing out all the vexation (Elder, 2010). In order to breakdown the frozen situation, Sammy may need to vocalize her feeling and telling her parents what she wants. Moreover, listening is one of the most important skills in communication. If Sammy can listen to the meanings behind the words, she may recognize many things. Conclusion In conclusion, Sammy may be suffering low self-concept with low self-esteem and identity crisis, low self-efficacy which causes low motivation and a poor family relationship. It is hoped that Sammy can deal with the problems and overcome the identity crisis, in consequent of having a valuing life. Reflection In the case of Sammy, she may just have a misunderstanding about herself and think that she is the worst in the world. Actually, I found that she also has some advantages. Especially since the patient just had surgery few days ago and could have some sort of life-threatening complications. Therefore, the next steps I would begin to take would be to reassess Mrs. P and begin asking her specific questions. According to Bickley (2021) I would pursue important details like asking her: “Where does the pain start?” “What is the pain like?” “How severe is it on a scale of 1 to 10?” “What makes it better or worse?” “Does it radiate or travel?” (p.300) Asking these specific questions regarding the location and radiation could give us a clue as to what the cause of her discomfort is. Following the reassessment and thorough questions, I would inform the patient that I am worried that this could be a complication from the surgery and in order to make sure she is safe and able to go home I need to inform the doctor so he can order labs and possible ultrasound. The reason why I would like to inform the patient about calling the doctor is because the patient told me this in confidence, therefore I wouldn’t want to ruin our relationship and cause her not to trust me in the future. At the same time making her understand that this could be something serious and that contacting the doctor is necessary. In an article by Ghalimah and Hamdi (2016), “postoperative cholecystitis should be promptly recognized because it is a life-threatening complication, with mortality ranging from 10% to 50%. The development of right upper quadrant abdominal pain associated with fever, tachycardia, and signs of peritoneal irritation after any major orthopedic procedures like: arthroplasty, spinal fusions, and others should alert the surgeon to the possibility of postoperative cholecystitis.” (Ghalimah & Hamdi, 2016) From another approach I would ask the patient if she has had any bowel movements since her surgery, or if she is passing any gas. The reason why this could be important is because the patient might simply just be constipated. Leonard (2018) wrote that, “many people experience constipation after surgery due to pain medications, anesthesia, or a lack of movement following the procedure.” (Leonard, 2018) Since Mrs. P just had the surgery few days ago, I’m sure she is still on certain pain medications, she is also probably not fully mobile, and her diet preoperatively and postoperatively has changed. Therefore, her body is going through a lot of changes and has undergone stress which has slowed down her bowel movements. It is important that the patient receives proper treatment in order to not develop other complications like hemorrhoids, anal fissure, fecal impaction, and rectal prolapse. All of this goes back to making sure we collect all the information preoperatively from the patient, as well as closely monitoring and constantly asking questions of our patients. Only then, in cases of emergency, will we be able to act quickly and providing proper treatment. Post 2: Karen English My patient, Mrs. P., is three days post op following a knee replacement when she confides in me that she feels bloated, nauseated and her belly hurts. As her nurse I would be concerned as that is not a normal finding after a knee replacement. I would start by asking the patient a few questions, “Did the pain start suddenly or gradually? Where does the pain start? Does is travel anywhere? What is her pain level on a scale from 1 to 10?” (Bickley, 2017). It would be necessary to determine whether this is a new complaint or something chronic that she has had prior to surgery. Since “patients cannot always clearly describe the location of pain in words…the quadrant where the pain is located helps identify the underlying organs that may be involved” (Bickley, 2017). Another important question to ask my patient would be when was her last bowel movement. After discussing these things with my patient, I would palpate her abdomen. First with one hand gently then more firmly with both hands focusing on how her abdomen presents, firm or soft, is she guarded and where the pain is located or if it’s more generalized over her entire abdomen. Next I would auscultate her abdomen in all four quadrants. I would be listen to her bowel sounds and take note if they are hypoactive. At this point I would also ask my patient if she has passed gas since having surgery. One study found that “when complications occur, post total knee arthroplasty, they tend to be cardiovascular and neurological events, rarely involving the gastrointestinal system” (Seah, et al., 2013). With this knowledge, it is important to find out how mobile she has been post surgery and how she has been dealing with the post surgical pain. In fact, opioid pain medication alone (not even in combination with any other factors) cause constipation in as many as 40% of patients. Educating the patient is of upmost importance to make sure the patient understands the role pain medication plays in post-op constipation. They often complain that they hadn’t been educated on the importance of stool softeners, increased mobility (not to exceed their limitations) and their diet and roles those play in their overall post surgery condition. Therefore, throughout the patient’s hospital stay, a focus on educating the patient on the proper diet, necessary mobility within their limits and pain medication instructions is very essential for more than adequate healing. This will also help the patient after discharge in their healing process.
