Applied Psychology and Criminology Assignment Case Study Report This essay will look at a biological and psychological theory to reach a diagnosis of a fictional case study. The aim of this essay will be to explain why twenty-seven year old Jack has the illness that he has and then a therapy will be presented that best fits Jack’s illness. From Jack’s paranoia that developed over a year working at the video store to the voices he heard and the strange comments that Jack made to his boss and his customers and ultimately his victim, it is apparent that Jack’s diagnosis could be schizophrenia. To give a more confident diagnosis of Jack, DSM edition five is used. In the DSM criteria for schizophrenia, it states that schizophrenia is defined by hallucinations, disorganized speech, delusions, social dysfunction and other symptoms. For a diagnosis of schizophrenia at least two of these symptoms have had to of been present in a individual for six months and the individual has have to of shown at least one month of active symptoms. (DSM-5.2013.) The case study shares that after a year of working at the video store, Jack quit his job after hearing voices and yelling at his boss as he believed that he was being watched. This shows that Jack was having hallucinations and delusions, which are two of the symptoms in the DSM-5 criteria. Not only this but Jack also began ‘talking strangely’ to customers which shows a third possible symptom of social dysfunction. (DSM-5.2013.) These three symptoms give a strong suggestion that Jack could have schizophrenia. A biological theory will be used to give the first explanation for the knife attack that Jack carried out on a stranger. The exact cause for schizophrenia is unknown but there are different contributing factors that are linked with the mental illness of schizophrenia (nhs.2012.) One main factor is genetics as schizophrenia tends to run in families. People can develop schizophrenia from a first degree family relation such as a parent or a sibling or a second degree relation such as a cousin or an aunt or uncle. However it is not the responsibility of one individual gene, it tends to be different combinations of genes that makes people more susceptible to schizophrenia. The case study doesn’t mention much about the extended family of Jack but it does mention that his uncle had spent a lot of time in the hospital and was being treated for verbal hallucinations. Although it doesn’t mention specifically as to whether Jack’s uncle had schizophrenia or another mental illness, hallucinations is one symptom of schizophrenia (nhs.2012.) and it is a symptom which Jack also suffered from so there is a possibility that it could of been passed down to him. Hallucinations also show that Jack is suffering from positive symptoms, something that a ‘sane’ individual wouldn’t normally have. (Applied Psychology and Criminology study session 2.) The criticism of this biological theory would be that the case study mentioned Jack occasionally used amphetamine which could have caused a dopamine in balance inside Jack’s brain and therefore caused him schizophrenia. But alternatively Jack’s use of amphetamine could have triggered the schizophrenia even though it may have possibly been genetically disposed to Jack. (Applied Psychology and Criminology study session 2.) The next theory is of a psychology nature and will give a second possible explanation for Jack’s illness. Expressed emotions or EE is a measure of the family environment. Although the case study doesn’t give a full insight into Jack’s home life or his childhood, it shows that his mum was always losing her temper and had a very neglectful and affectionless nature towards Jack and it briefly mentions how he enjoyed school because this meant he got to get away from his parents. It also mentions how he always found his home life very volatile. EE’s measurement of the family environment includes the quality of interaction of family relationships among family members and patients of schizophrenia. (Anekal.C. et al.2012.)The case study only mentions a volatile and unhappy relationship with his mother but it does mention that he liked going to school because he could get away from his parents, so it would suggest that a relationship with his father or father figure was the same as the one he had with his mother; volatile and unhappy. A high level of EE in the home can cause an outcome of someone developing schizophrenia or another mental illness. In Jack’s case it is possible to suggest that the unstable home life he had would have contributed to a high level of EE and therefore contributed to his mental illness. Individuals that have a high level of EE tend to have hostile and judgemental characteristics. On the other hand low expressed emotion is when individuals are more reserved and not judgemental. (Anekal.C. et al.2012.) To a great extent the biological explanation is successful in explaining Jack’s disorder. The idea that genetics could be a possible reason behind Jack’s suggested diagnosis is a clear and confident explanation as there is evidence of genetic disposition from the case study. His uncle visited the hospital numerous times with hallucinations and other schizophrenia related symptoms, and although his mum didn’t have a mental illness diagnosis, her characteristics were not of a motherly nature. A study that would support the biological theory of genetics is the study of monozygotic twins. Monozygotic twins or identical twins have the same genetic makeup, with identical twins it is expected that if one has a particular trait through genetic cause then the other would have the same trait, along with other factors the risk of schizophrenia in monozygotic twins is between forty and sixty percent compared to non-identical twins at ten to twenty percent, although Jack didn’t have a twin it gives a insight into percentages of mental illnesses and family relations. (Goldberg.E.T et al.ND.) To a great extent the psychological explanation of expressed emotions is also successful in explaining Jack’s mental illness. The expressed emotion theory looks at measuring the home life of an individual and how this could contribute to an individual developing a mental illness. The case study of Jack gives us a clear indication of Jack’s unstable home life and this shows that there was a high level of EE, this too could have contributed to Jack’s possible schizophrenia. A study to support this psychological explanation is interpersonal relations by Bateson. Bateson, 1972 paid attention to family relations and the distinctive system of interpersonal relationships where individual’s lives unravel. He looked at how individuals communicate through communication and how certain expressions were very significant. Jack’s case study gives a clear insight into his home life and relationships with his family.(Ceryone.D et al. ND.) In Jack’s case the most obvious and successful treatment would be antipsychotic medication which can effectively treat schizophrenia however it came about. Individuals have roughly one out of a four chance that symptoms will not return if they have suffered just one schizophrenic episode. Jack has suffered for a year with schizophrenic symptoms so it is likely that Jack would need to carry on taking antipsychotic medication for the foreseeable future. (Royalcollegeofpsychiatrists.ND.) Some side effects of taking these antipsychotic medication is sleepiness, dizziness, increased chance of developing diabetes and weight gain however these are only possible symptoms and Jack may not suffer from some if any of them. In conclusion, both the biological and psychological explanations give a successful insight into what could have caused Jack’s possible schizophrenia, and neither is better than the other. The idea of genetics can fully relate to Jack as even though there was no diagnosis, both his mother and his uncle weren’t in a sane state of mind, which could have easily been passed onto Jack. Expressed emotions links to Jack as it explains how his turbulent and volatile home life could have too easily made Jack more susceptible to schizophrenia. In terms of Jack’s possible treatment, antipsychotic medication seems to be the most successful way of treating him. The statistics show how well it works for individuals with schizophrenia and the chances of his symptoms coming back and having terrible side effects are slim. However even though medication seems to be the most successful treatment, from the case study Jack should be having a combination of treatments as well as the antipsychotics. This could include some form of therapy such as family therapy where the mother and uncle could be involved to try and rebuild relationships or one on one counselling. The order in which you sit or stand will provide messages to the person you are communicating with. Sitting with your arms crossed can mean ‘I’m not taking any notice’ which will give a negative effect to the person you are trying to communicate with. However if you lean forward it will pursue that you are showing interest or intense involvement. A good posture is needed to influence a person as to create a good reaction about you. In a health and social care environment such as a day care centre you will need to sit or stand in a position which the person you are communicating with feels comfortable as this will be necessary to show a good impression. For example if you are having a one-to-one talk in a day care centre you will need to show you have a positive posture as this constructs a positive form of response to the elderly person you are talking to and get down onto their level so they feel relaxed communicating with you. Gestures: Gestures may include hand and arm movements which can help us to understand what a person is saying. A hand gesture may involve bad/good signals such as friend/not friend and also the middle finger which is known as swearing to the young generation. The gestures matter to a person as in order to communicate this will need to be on point as it includes positive/negative impressions to a person. Within a health and social care setting, gestures are important to get right as a location such as a hospital you will need to have a formal atmosphere which includes gestures such as hands showing where things may be or even helping an elderly/child/adult to move. Facial expressions: The faces you make indicate your emotional state. If a person is in a sad mood they can show this emotion by looking down this may show tension in their face so their mouth will be closed. The best way to let a person feel you are involved with them is to look at them while they speak and not invade their personal space. If one has their back to them with no eye contact in a health and social care environment this can create a conversation with people behind you to talk about you behind your back or even show negative hand gestures. To overcome this barrier there are many strategies some of the ways are: If a hearing aid does not create a big difference to an individual’s hearing the other option is to use: British Sign Language: This is a strategy in which people in Britain communicate with deaf people. The British Deaf Association had explained that British Sign Language is the first or most preferred language of many deaf people in the United Kingdom. Sign language is a way to overcome the barrier as this strategy makes deaf people understand what we are saying and what they are saying to, however you will need to know how sign language is worked and know all the background behind it and you will need experience to do this before starting a conversation with a deaf person otherwise you may send a wrong message to them. In a health and care environment such as a residential care home the British Sign Language may be a good way to communicate as some may be hard at hearing and will need to communicate in such a way that both people understand. Also within a health and social care setting such as a hospital there may be patients that are deaf, to overcome this the nurses, receptionists and staff within the environment should be aware of this barrier and will need to be taught this before working as this will be a advantage as the form of communication will rise between the two that are communicating. Blindness: This means when a person has lack of vision which means they are unable to see. A person can use glasses or even contact lenses as they improve the vision, however if this does not help one can use what is called Braille to overcome the barrier of being blind. Braille: This is a system which can be felt with fingers. This system provides a means of written communication which is based on the sense of touch, for people that have limited vision. The system is popular for reading and writing by the people that cannot see written script. Computer software can also translate written pieces into Braille. This is helpful to communicate with those that have difficulties with their eye sight as this makes them feel as if they have a chance to communicate in a special way to others. Within a health and social environment it may be essential to use Braille as this helps the people with lack of eye sight; this gives them a hope to communicate in a unique way and is a way to regain their eye sight. However the other option is: Makaton: This is a system that develops language which uses speech, signs and symbols to help people with learning difficulties to communicate and to develop their language skills. Those that communicate using Makaton can speak a word and perform a sign using hands and body language. There are many symbols which can help people that experience learning difficulties to recognise an idea or to communicate with others. Makaton is tremendously flexible as it can be fixed to an individual’s needs and used at a level suitable for them. It can also be used to share thoughts, choices and emotions, help people find their way and many more. This is also useful in a health and care environment as people that suffer learning difficulties suffer using speech but this system develops the learning and will be easier for people that communicate with them. Within a school environment if a child already knows sign language this will be useful however the rest may feel insecure as they do not know how to communicate within that certain way which will decrease their communication within the environment. Lastly the areas of improvement for the barrier British Sign Language is that people in primary and secondary school should be able to get a chance to learn this method as this may be an advantage to them in the future if they would like to work within a health and social care environment that also takes in people with less hearing. The strategy of this can be improved by getting a range of sign languages so that it is more understandable for people that are not suffering from being deaf. Learning difficulties- Makaton: Makaton is a barrier which strengths is that it is different from British Sign Language because it is a language programme which balance verbal communication with actions and other non verbal signs. Makaton consists a core vocabulary of particularly selected concepts and ideas. These are considered to be most appropriate for the needs of children and adults with communication and language difficulties. The choice of the process relies on the needs of the child or adult with communication problems and main people who play an important role in their lives. This is an advantage for a health and social care environment as here may be people that lack in speech and this system will be a good programme for them to develop for them.

Political, economic, and social characteristics affect how much a country spends on healthcare. However, the wealthier the country, the tendency is to spend more on healthcare. For example, “The U.S. spends the United States spent about $11,100 per person on healthcare — the highest healthcare cost per capita across the OECD. For comparison, Switzerland was the second highest-spending country with about $7,700 in healthcare expenses per capita, in 2019.” (Peter Peterson Foundation, 2020). The average is $5,496. Discussion The U.S healthcare system has been reformed due to the implementation of the Affordable Care Act of 2010. The purpose of the ACA of 2010 was to reduce the cost of healthcare in the U.S. and ensure that everyone is covered under a healthcare plan that is affordable for everyone. (ACA of 2010, 2010). The question is if it is addressing the healthcare needs of the citizens? I can honestly say that it does not address all of the citizen’s needs in my real everyday life. I have many friends and family who cannot afford health insurance, even those listed in the Marketplace. On the other hand, I also have friends and family with the Marketplace insurance plan, and they do not cove very many services or lab tests. In other experiences, I have family members that we can obtain health care services and medical care that without the ACA of 2010, they would not normally be able to afford or receive. In some cases, like my mom, she would not qualify for Medicaid as it stands today without the ACA of 2010. So, I have experienced it meeting the needs and not meeting the needs. People with pre-existing conditions can not be denied healthcare, which is definitely a plus for the ACA of 2010. (ACA of 2010, 2010). In my opinion, keeping dependent children on the parent’s insurance until they are 26 is a benefit. The expansion of Medicaid is a benefit, and healthcare is offered to everyone is a benefit. Still, the program needs a little more tweaking. Where do we as a country go from here? What do we need to do to improve the law? First, the U.S. as a country needs to work together and look at the average income of the citizens and find ways to cut back the cost of coverage and expensive lab tests. Second, we need to find ways to control pharmaceuticals’ substance abuse and high costs and hold accountable the pharmaceutical companies and doctors that overprescribe opioids and other strong drugs. Finally, let us go back to eating non-processed foods and exercise more to get healthier as a nation. We need to go back to playing baseball in the streets and other sports and exercise and set the right examples for the younger generation. The fast foods and unhealthy food businesses need to be higher in price, and we need to lower the price on the healthier foods and snacks and the healthier choices. There are just so many other things to go from here; we need to discuss and put them into action. ConclusionThe U.S. needs to find different solutions to reduce healthcare spending for all involved and promote healthy eating and exercise. Reducing the prices on healthier things and what people need and raising prices on unhealthy things will be a great start. Healthy food and exercise is awesome. Eating farm to table food, and less processed food is even better.
